Comparison of Cardiopulmonary Exercise Test with Severity in COPD According to GOLD Classification

Abstract
Chronic obstructive pulmonary disease (COPD) is characterized by an incompletely reversible limitation in airflow. Resting pulmonary function test have been considered to be the ¡°gold standard¡± as a diagnosis and severity evaluaton in COPD. But resting pulmonary function test, presented as FEV1, have not the information about exercise performance and exercise-induced hypoxemia in COPD patients. Therefore, the aim of this study was to compare the exercise function by cardiopulmonary exercise test (CPET) with the severity in COPD patients using the Global Initiative for Chronic Obstructive Lung Disease (GOLD) classification. This study examined 98 patients with an airway obstruction. mean age was 61.5 ¡¾ 15 years. The mean smoking history was 40 ¡¾ 20 pack years. The patients underwent spirometry and CPET, and the severity of the airflow obstruction was classified by GOLD. There were 30 patients on stage I and 42 patients on stage II, 20 patients on stage III, 6 patients on stage IV. The GOLD classification was correlated with VO2max and VEmax and Breathing reserve (p < 0.05). The maximal oxygen uptake was significantly lower at more advanced stage. Anaerobic threshold detection rate was decreased at advanced stage. But VO2max was more correlated with FEV1 (r = 0.715, p < 0.001)) than GOLD classification (r = ?0.402, p < 0.05). So the GOLD classification may be not enough to reflect the exercise capacity.
